Any provider licensed or certified under the laws of the Commonwealth whose license or <br>certificate to practice is suspended, revoked, limited, or otherwise restricted shall have <br>that fact reported to the medical assistance program by the respective licensure or <br>regulatory board or agency within five (5) working days of the action. Effective: July 15, 1994 <br>History: Created 1994 Ky. Acts ch. 96, sec. 15, effective July 15, 1994; and ch. 316, sec. 15, effective July 15, 1994. Legislative Research Commission Note (7/15/94).
