On or before the end ofJanuary, April, July, and October of each year, each surplus lines licenseeshall file with the Commissioner, on a form prescribed by the Commissioner, averified report of all surplus lines insurance transacted during the precedingthree months showing:
(1) Aggregate grosspremiums written;
(2) Aggregate returnpremiums; and
(3) Amount of aggregatetax to be remitted. (1985, c. 688, s. 1; 1987, c. 864, s. 36.)
