Statutes > New-hampshire > TITLEXXXIV > CHAPTER376 > 376-9Applications for certificates and permits required by RSA 376:4 and RSA 376:7 shall be:
I. Made in writing to the department;
II. Verified under oath;
III. Accompanied by the proper fee;
IV. In the form specified in rules adopted by the commissioner pursuant to RSA 541-A; and
V. With the information specified in rules adopted by the commissioner pursuant to RSA 541-A.
