The application for a notarypublic commission shall contain the following declaration to be executed byeach applicant under oath:
Declaration of Applicant
I, ___________________ (name ofapplicant), solemnly swear or affirm under penalty of perjury that theinformation in this application is true, complete, and correct; that Iunderstand the official duties and responsibilities of a notary public in thisState, as described in the statutes; and that I will perform to the best of myability all notarial acts in accordance with the law.
