§55A‑1‑21.  Forms.

(a)        The Secretary ofState may promulgate and furnish on request forms for:

(1)        An application for acertificate of existence;

(2)        A foreigncorporation's application for a certificate of authority to conduct affairs inthis State;

(3)        A foreigncorporation's application for a certificate of withdrawal;

(4)        Designation ofPrincipal Office Address; and

(5)        Corporation'sStatement of Change of Principal Office.

If the Secretary of State sorequires, use of these forms is mandatory.

(b)        The Secretary ofState may promulgate and furnish on request forms for other documents requiredor permitted to be filed by this Chapter but their use is not mandatory. (1955,c. 1230; 1993, c. 398, s. 1; 1995, c. 539, s. 9.)
