Every person fitting andselling a hearing aid, be it new or used, in the State of North Carolina, atthe time of delivery of the hearing aid shall render to the user and/orpurchaser a statement of sale to include the following:
(1) Date of delivery
(2) Condition of hearingaid; new, used, reconditioned
(3) Hearing aididentification number
(4) Name of manufacturer
(5) Price of hearing aid
(6) Charge for fittingand service
(7) Name of dealerand/or fitter
