44-1744. Contract; additional provisions
required

Every contract for dance studio lessons and other services shall provide:

1. That the lessons will begin within thirty days.

2. That a contract with respect to new or increased dance studio lessons and other
services, which are not subject to prior agreements between the parties, may be canceled
within fifteen days after the date of receipt by the customer of a copy of the contract
by written notice to the other party by registered mail return receipt requested at the
address specified in the contract. All moneys paid pursuant to such contract shall be
refunded within ten days of receipt of the notice of cancellation except that payment
shall be made for any dance studio lessons and other services received prior to such
cancellation.

3. A written statement of the hourly rate for the lessons and other services.

4. A conspicuous notice as follows:

"Notice to Customer

Do not sign this contract if any of the spaces intended for the agreed terms to the
extent of then available information are left blank. You are entitled to a copy of this
contract at the time you sign it.

You may cancel this contract within fifteen days of the time you receive a copy by
written notice to the dance studio by registered mail return receipt requested at the
address specified in the contract.

If you cancel the contract in the fifteen-day period, you are entitled to a refund
of your money less payment for dance lessons or services received before
cancellation. Refunds must be made within ten days, not including Sundays and holidays
of receipt of the cancellation notice by the studio."
