A claimant's affidavit may be in the following form, viz.:

D. ________ , and now in the hands of ________ , a ________ of ________ county, under said execution, is the property of affiant, and is not the property of ________ .
"Sworn to and subscribed before me, ________ , A. D. ________ ________ , J. P."
