(a) A document requiredor permitted by this Act to be filed by the Secretary of State must be filedunder Chapter 55D of the General Statutes.
(b) A documentsubmitted for filing by the Secretary of State on behalf of a generalpartnership must be executed by a general partner of the partnership.
(c) The Secretary ofState may adopt and furnish on request forms for:
(1) An application forregistration as a registered limited liability partnership;
(2) Cancellation ofregistration as a registered limited liability partnership;
(3) Application forregistration as a foreign limited liability partnership; and
(4) Cancellation ofregistration as a foreign limited liability partnership.
If the Secretary of State sorequires, use of these forms is mandatory.
