Statutes > Connecticut > Title19a > Chap368v > Sec19a-505aSec. 19a-505a. Hospital to provide forms for birth certificate and affidavit of parentage to parents of child born out of wedlock. Upon the birth of a child born out of wedlock, the hospital or such other institution where such birth occurs shall provide an opportunity for the mother and putative father to complete a birth certificate and an affidavit of parentage.
