§441-29  Application for license; fees. 
Every applicant for a license under this chapter shall file an application with
the director in such form and setting forth such information as may be
prescribed or required by the director and shall furnish such additional
information bearing upon the issuance of the license as the director requires. 
In the case of a co-partnership or corporation any officer may sign the
application on behalf of the applicant.  The application shall be accompanied
