§489D-10  Application and license fees. 
(a)  Each application shall be accompanied by:



(1)  A nonrefundable application fee in the amount of
$2,000 plus $300 for each additional location in the State, not to exceed an
aggregate fee of $15,000; and



(2)  An annual license fee of $2,000 plus $300 for
each additional location in the State, not to exceed an aggregate fee of $15,000.



(b)  The license fee shall be refunded if the
