§514A-32  Questionnaire and filing fee. 
The notice of intention shall be accompanied by:



(1)  A nonrefundable fee as provided in rules adopted
by the director of commerce and consumer affairs pursuant to chapter 91;



(2)  A copy of a questionnaire properly filled in; and



(3)  Such documents and information concerning the
project as may be specified by the commission.



The questionnaire shall be in such form and content
