§88-44  Enrollment.  Upon entering orreentering service, an employee shall file with the board such information asthe board of trustees may require for enrollment and administrative purposesincluding a designation of a person or persons to receive any benefit that maybe payable in the event of the employee's death.

An employee becoming a member of the systemshall also present at such time and in such form as the board prescribes,evidence of the employee's date of birth.  No statement or record of age orbirth made or presented by a member of the system may be impeached by themember or the member's successors in interest.
