[§327K-1]  Definitions.  As used in thischapter, unless the context otherwise requires:

"Department" means the department ofhealth.

"Form" means a physician orders forlife-sustaining treatment form adopted by the department.

"Health care provider" means anindividual licensed, certified, or otherwise authorized or permitted by law toprovide health care in the ordinary course of the individual's business orprofession.

"Patient's physician" means aphysician licensed pursuant to chapter 453 who has examined the patient.

"Physician orders for life-sustainingtreatment form" means a form signed by a patient, or if incapacitated, bythe patient's surrogate and the patient's physician, that records the patient'swishes and that directs a health care provider regarding the provision ofresuscitative and life-sustaining measures.  A physician orders forlife-sustaining treatment form is not an advance health-care directive.

"Surrogate" shall have the samemeaning as in section 327E-2. [L 2009, c 186, pt of §1]
