§412:13-216  Representative office;application.  (a)  The application for a license to establish and maintaina Hawaii representative office shall be in writing under oath and shall be in aform and contain any information as the commissioner may require by rule ororder.  The application shall be accompanied by a nonrefundable application feeof $1,500 or a greater amount as the commissioner may establish by rule.

(b)  An application to establish and maintain aHawaii representative office shall include, if applicable, a statement underoath appointing an agent in this State for receipt of service of process inaccordance with section 414-437, if the license is granted.
