[§414D-4]  Forms.  (a)  Thedepartment director may prescribe and furnish on request, forms for:

(1)  An application for a certificate of goodstanding;

(2)  A foreign corporation's application for acertificate of authority to transact business in this State;

(3)  A foreign corporation's application for acertificate of withdrawal; and

(4)  The annual report.

If the departmentdirector so requires, use of these forms is mandatory.

(b)  The departmentdirector may prescribe and furnish on request forms for other documentsrequired or permitted to be filed by this chapter but their use is notmandatory. [L 2001, c 105, pt of §1]
