§431:8-306  Signature of broker and specialendorsement of surplus lines policy.  Every insurance contract procured anddelivered as a surplus lines coverage pursuant to this part, including anyevidence of insurance other than a policy, shall:

(1)  Bear the name and address of the surplus linesbroker who procured it, and

(2)  Have stamped or written conspicuously upon thefirst page of the contract the following:

"This insurance contract is issued byan insurer which is not licensed by the State of Hawaii and is not subject toits regulation or examination.  If the insurer is found insolvent, claims underthis contract are not covered by any guaranty fund of the State ofHawaii." [L 1987, c 347, pt of §2]
