§432D-17  Fees.  (a)  The commissionershall collect in advance the following fees:

(1)  For filing an application for a certificate ofauthority or amendment thereto, $600; and

(2)  For all services subsequent to the issuance of acertificate of authority (including extension of the certificate of authority),$400.

(b)  The commissioner shall notify the holderof the certificate of authority by written notice at least thirty days prior tothe extension date of the certificate.  If the fee is not paid before or on theextension date, a penalty shall be imposed in the amount of fifty per cent ofthe fee.  If the fee and the penalty are not paid within thirty daysimmediately following the extension date, the commissioner may revoke thecertificate of authority and shall not reinstate the certificate of authorityuntil the fee and penalty have been paid.

(c)  All fees and penalties collected pursuantto this section and penalties collected pursuant to section 432D-14 shall beremitted by the commissioner to the director of finance and shall be placed tothe credit of the general fund. [L 1995, c 179, pt of §1; am L 1999, c 247, §2]
