IC 25-17.6-5-1 Expiration of license
Sec. 1. A license issued under IC 25-17.6-4 expires three (3) years
after it is issued, at a time and date designated by the board. As added by P.L.161-1996, SEC.2. Amended by P.L.17-1999,

IC 25-17.6-5-2 Notification of expiration and renewal fee
Sec. 2. The board shall notify each person licensed under this
article of the:
(1) date of expiration of the person's license; and
(2) amount of the renewal fee established by the board. As added by P.L.161-1996, SEC.2. Amended by P.L.17-1999,

IC 25-17.6-5-3 Failure to pay renewal fee
Sec. 3. If a person fails to pay the renewal fee within ninety (90)
days after the expiration date established by the board under this
chapter, the person's:
(1) license shall be suspended; and
(2) name shall be deleted from the roster described in
IC 25-17.6-3-10;

IC 25-17.6-5-4 Revocation of license for failure to pay renewal fee; recertification
Sec. 4. (a) If a person fails to pay the renewal fee for two (2)
consecutive years, that person's license shall be revoked and may not
be renewed.
(b) If a person whose license has been revoked under subsection
(a) desires to be relicensed, the person:
(1) must submit an application to the board; and
(2) shall be considered as a new applicant. As added by P.L.161-1996, SEC.2. Amended by P.L.17-1999,
