6B.9 FORM OF NOTICE -- SIGNATURE.
1. Said notice shall be in substantially the following form, with
such changes therein as will render it applicable to the party giving
and receiving the notice, and to the particular case pending, to wit:

other persons, companies, or corporations having any interest in or
