501A.206 FORMS. 1. The secretary may prescribe and furnish on request forms, including but not limited to the following: a. An application for a certificate of existence. b. A foreign cooperative's application for a certificate of authority to transact business in this state. c. A foreign cooperative's application for a certificate of withdrawal. If the secretary so requires, use of these listed forms prescribed by the secretary is mandatory. 2. The secretary may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter, but their use is not mandatory.
