Chapter 65.--PUBLIC HEALTH
Article 51.--HOME HEALTH AGENCIES

      65-5103.   Application for license; annual fee.(a) Persons desiring to receive a license to operate a home health agency shallfile a written application with the secretary on a form prescribed by thesecretary.

      (b)   The application shall be accompanied by a license fee fixed by rulesand regulations of the secretary which establish a sliding scale for suchfee based upon the actual or estimated number of home health visits to beperformed by a home health agency during a calendar year. The initiallicense fee may be greater than subsequentannual fees as established by the secretary by rules and regulations.
