Chapter 65.--PUBLIC HEALTH
Article 61.--EMERGENCY MEDICAL SERVICES

      65-6127.   Permit to operate ambulance service;application; contents.(a) Application for a permit to operate an ambulance service shall be madeto the board by the operator of the ambulanceservice upon forms provided by the administrator and shall be accompaniedby a permit fee which shall be a base amount plus an amount for eachvehicle used by such operator in such operator's ambulance service andwhich shall be fixed by rules and regulations of the board to cover all orany part of the cost of regulation of ambulance services.

      (b)   The application shall state the name of the operator, the names of theattendants of such ambulance service, the primary territory for which thepermit is sought, the type of service offered, the location and physicaldescription of the facility whereby calls for service will be received, thefacility wherein vehicles are to be garaged, a description of vehicles andother equipment to be used by the service and such other information asthe board may require.

      (c)   Nothing in this act shall be construed as granting an exclusiveterritorial right to operate an ambulance service. Upon change of ownershipof an ambulance service the permit issued to such service shall expire 60days after the change of ownership.
