Marriage dissolution or annulment record.

193.205. 1. A record of each dissolution of marriage and annulmentof marriage granted by any court in this state shall be filed by the clerkof the court with the department and shall be registered if it has beencompleted and filed in accordance with this section. The record shall beprepared by the petitioner or such petitioner's legal representative on aform prescribed and furnished by the state registrar and shall be presentedto the clerk of the court with the petition.

2. The clerk of the court shall complete and forward to thedepartment on or before the fifteenth day of each calendar month therecords of each dissolution of marriage and annulment of marriage decreegranted during the preceding calendar month.
