Application for license, contents--fee.

197.050. Application for a license shall be made to the department ofhealth and senior services upon forms provided by it and shall contain suchinformation as the department of health and senior services requires, whichmay include affirmative evidence of ability to comply with such reasonablestandards, rules and regulations as are lawfully prescribed hereunder. UntilJune 30, 1989, each application for a license, except applications fromgovernmental units, shall be accompanied by an annual license fee of twohundred dollars plus two dollars per bed for the first one hundred beds andone dollar per bed for each additional bed. Beginning July 1, 1989, eachapplication for a license, except applications from governmental units, shallbe accompanied by an annual license fee of two hundred fifty dollars plusthree dollars per bed for the first four hundred beds and two dollars per bedfor each additional bed. All license fees shall be paid to the director ofrevenue and deposited in the state treasury to the credit of the generalrevenue fund.
