Form of ballot--vote necessary to adopt.

205.020. 1. The question shall be submitted insubstantially the following form:

Shall there be a maximum tax of ...... per hundred dollarsassessed valuation for a county health center and the maintenanceand operation of same?

2. If a majority of the votes cast on the question shallvote in favor of such tax, the county commission shall proceed tolevy and collect such tax and deposit same in the county treasuryto the credit of the health center fund and such fund shall beexpended as hereinafter provided.
