209.070. It shall be the duty of the division of familyservices to prepare suitable blank application forms for the useof blind persons in making application for pensions, which shallcontain such questions for applicant to answer and other matteras the division may deem appropriate to the end to beaccomplished. All statements of an applicant contained on suchapplication form shall be verified by the applicant and shallalso be supported by the certificates of two disinterested andresponsible householders of the county wherein applicant resides,who have known applicant for not less than two years next priorto date of such application, that such statements are true.
