Access to records on the suspension or revocation of a foster homelicense--procedure for release of information.

210.498. Any parent or legal guardian may have access toinvestigation records kept by the division regarding a decision for thedenial of or the suspension or revocation of a license to a specific personto operate or maintain a foster home if such specific person does or mayprovide services or care to a child of the person requesting theinformation. The request for the release of such information shall be madeto the division director or the director's designee, in writing, by theparent or legal guardian of the child and shall be accompanied with asigned and notarized release form from the person who does or may providecare or services to the child. The notarized release form shall includethe full name, date of birth and Social Security number of the person whodoes or may provide care or services to a child. The response shallinclude information pertaining to the nature and disposition of any denial,suspension or revocation of a license to operate a foster home. Thisresponse shall not include any identifying information regarding any personother than the person to whom a foster home license was denied, suspendedor revoked. The response shall be given within ten working days of thetime it was received by the division.
