210.501. 1. Application for license shall be made on formssupplied by the division and in the manner prescribed.
2. Each license shall state clearly the name of the fosterhome, residential care facility, or child placing agency, itslocation, the kind of program the licensee is permitted toundertake, and the number, age range, and other pertinentinformation concerning children who may be served.
