Annual reports by certain licensees--contents.

313.045. Each licensee which awards to winners of bingogames prizes or merchandise having an aggregate retail value ofmore than seven thousand five hundred dollars annually shallreport annually to the commission on forms prescribed by thecommission the following information:

(1) The number of games it has conducted during thereporting year;

(2) The location at which and the days it conducted games;

(3) The gross receipts it received from each game;

(4) An itemization of the cost of conducting each game,other than for prizes, and the names and addresses of the personto whom said expenses were paid;

(5) The purposes for which the net proceeds of the gamewere used and the amounts so used;

(6) Any other information that the director may require byrule or regulation.
