Application, form, fee.

327.141. Applications for examination and license as an architectshall be typewritten on prescribed forms furnished to the applicant. Theapplication shall contain the applicant's statements showing theapplicant's education, experience, results of previous architecturallicensing examinations, if any, and such other pertinent information as theboard may require. Each application shall contain a statement that it ismade under oath or affirmation and that its representations are true andcorrect to the best knowledge and belief of the person signing theapplication, subject to the penalties of making a false affidavit ordeclaration and shall be accompanied by the required fee.
