Application, form, fee.

327.231. Applications for examination and license as a professionalengineer shall be typewritten on prescribed forms furnished to theapplicant. The application shall contain the applicant's statementsshowing the applicant's education, experience, results of previousengineering examinations, if any, and such other pertinent information asthe board may require. Each application shall contain a statement that itis made under oath or affirmation and that its representations are true andcorrect to the best knowledge and belief of the person signing suchapplication, subject to the penalties of making a false affidavit ordeclaration and shall be accompanied by the required fee.
