Application, form, content, oath or affirmation of truth, penaltiesfor making false affidavit, fee.

327.615. Applications for examinations and licensure as a landscapearchitect shall be typewritten on forms approved by the board. Theapplication shall contain the applicant's statements showing theapplicant's education, experience, results of previous landscapearchitectural licensing examinations, if any, and such other pertinentinformation as the board may require. Each application shall contain astatement that it is made under oath or affirmation and that itsrepresentations are true and correct to the best knowledge and belief ofthe person signing the application subject to the penalties of making afalse affidavit or declaration, and shall be accompanied by the requiredfee.
