332.241. Applications for examination and registration as adental hygienist shall be in writing on forms furnished to theapplicant which shall contain applicant's statements showing hiseducation and such other pertinent information as the board mayrequire. Each application shall contain a statement that it ismade under oath or affirmation and that its representations aretrue and correct to the best knowledge and belief of the personsigning same, subject to the penalties of making a falseaffidavit or declaration, and shall be accompanied by a dentalhygienist examination fee.
