Applications, form, contents, fees.

336.040. Every person who desires to obtain a license to practiceoptometry shall apply to the board, in writing, on forms prepared andfurnished by the board. The application shall state the requiredqualifications of the applicant, that the representations are made underoath or affirmation and are true and correct to the applicant's bestknowledge, subject to the penalties of making a false affidavit ordeclaration and shall be accompanied by the examination fee.
