Information to be available to enrollees.

354.440. Every health maintenance organization shall makeavailable to its enrollees:

(1) The most recent annual statement of financial condition,including a balance sheet and summary of receipts anddisbursements;

(2) A description of the organizational structure andoperation of the health care plan and a summary of any materialchanges since the issuance of the last report;

(3) A description of services and information as to whereand how to secure them; and

(4) A clear and understandable description of the healthmaintenance organization's method for resolving enrolleecomplaints.
