Proposed provider contract forms filed with the director--contractsmaintained at place of business, available for review, when.

354.624. 1. A health carrier shall file with the director allcontract forms proposed for use with its participating providers andintermediaries. The forms shall not contain any information oncompensation terms, rates or other payments by the carrier to participatingproviders or intermediaries and shall contain information on any terminvolving risk-sharing arrangements between the parties.

2. The health carrier shall maintain provider and intermediarycontracts at its principal place of business in the state, or the healthcarrier shall have access to all contracts and provide copies to facilitateregulatory review upon twenty days' prior written notice from the directoror at the time of any examination.
