Application for writ.

491.250. An application for such writ shall be verified byaffidavit, and shall state the title and nature of the proceedingin which the testimony of the prisoner is desired, the court orofficer before whom pending, and that the testimony of suchprisoner is material and necessary to the applicant on the trialor hearing of such suit or proceeding, as he is advised bycounsel, and verily believes.
