Department to release information to next of kin and personsresponsible for costs, when.

630.145. 1. Notwithstanding the provisions of section630.140, a residential facility or day program operated, fundedor licensed by the department may release to a patient's orresident's next of kin, attorney, guardian or conservator, ifany, the information that the person is presently a patient,resident or client in the facility or program, or that the personis seriously physically ill, and shall notify a voluntarypatient's or resident's next of kin, attorney, guardian, orconservator or any other person who may be responsible for thecosts incurred by such patient or resident, of the admittance ofsuch patient or resident.

2. Upon the death of a patient or resident, the facilityshall notify his next of kin, guardian or conservator, if any,about the death and its cause.

3. Next of kin shall be notified under this section in thefollowing order unless otherwise indicated by the patient orresident:

(1) Spouse;

(2) Parents;

(3) Children;

(4) Brothers and sisters;

(5) Other relatives according to the degree of relation.

4. The patient or resident may indicate additional personswhom he wants notified in the event of his death or seriousinjury or incapacity.
