   I. Within 7 days after the final decree is filed, the register of probate shall send a hard copy of the report of the adoption to the town clerk of the town where the adoptee was born, to the commissioner, and to the department of state, division of vital records administration. The department of state, division of vital records administration shall provide suitable forms for such reports.
   II. Each month the register of probate shall send to the department, on forms supplied by the department, summaries of the affidavits filed pursuant to RSA 170-B:19, V. The department shall publish an annual report relative to general fees and other charges for adoption and related services.
