Every application for a certificate of registration for ophthalmic dispensing shall be accompanied by a non-refundable registration fee as determined by the commissioner. Upon approval of the application by the commissioner, the applicant shall be issued a certificate of registration for ophthalmic dispensing to be valid for 2 years. The fee for renewal of any certificate of registration shall be determined by the commissioner.
