   I. Regulated persons and entities, including but not limited to licensees, insurance companies, health care organizations, and health care facilities, shall report to the board any conviction by a court of law or determination by an agency that a licensee has committed an act that constitutes a violation of this chapter.
   II. Persons and entities shall be immune from civil liability, whether direct or derivative, for providing information in good faith to the board pursuant to paragraph I.
   III. The board shall not disclose the identity of a person or entity who provides information unless disclosure is required by disciplinary proceedings or a court of law.
