SECTION 28-43-6

   § 28-43-6  Notification of benefit paymentsto employers. – (a) Within thirty (30) days after the end of each calendar quarter, or morefrequently if deemed necessary, the director shall furnish to each employer whois charged for benefits paid to claimants under this title during that quartera notice which shall contain at least the following information:

   (1) The name and social security number of the payee;

   (2) The amount paid;

   (3) The date on which that payment was made;

   (4) The calendar week for which that payment is made; and

   (5) The name of the chargeable employer and his or herregistered account number.

   (b) That notification to an employer shall constitute thedirector's determination and shall be binding upon the employer unless anappeal is duly filed within fifteen (15) days of its mailing.
