3-7-16. Disciplinary action--Notice and filing of statement. In every case of disciplinary action, a statement shall be made in duplicate and shall be signed by the appointing officer. One copy of the statement shall be delivered personally or by certified or registered mail to the employee, and the other copy shall be filed for the record.
