3-12A-23. Certificates issued to individual employees. Each employee who is covered under any contract authorized under § 3-12A-5.1 shall receive a certificate setting forth the benefits to which the employee and the employee's dependents are entitled under this chapter, to whom the benefits are payable, to whom claims shall be submitted, and a summary of the provisions of the contracts as they affect the employee and the employee's dependents. The certificate is in lieu of the certificate which the insurance carrier issuing the contract would otherwise issue.
