[§286-116.5]  Notice of change of address orname; penalty.  (a)  If the residence address of an applicant for, or aholder of, a driver's license is changed from that stated in the application orin the license issued to the applicant or holder, the person shall, withinthirty days after such change in address, notify the examiner of drivers inwriting of the person's old and new residence addresses and the number of anylicense then held by the person.

(b)  If the name of an applicant for, or aholder of, a driver's license is changed from that shown on the applicant's orholder's application or license, the person shall, within thirty days after thechange of name, notify the examiner of drivers in writing of the person'sformer name and the new name and of the number of any permit or license thenheld by the person.  The examiner of drivers may require the person to filesatisfactory proof of the change of name.

(c)  Any person who violates this section shallbe fined not more than $25. [L 1975, c 43, §2; gen ch 1985]
