    Chapter 3. Temporary Assistance for Needy Families; Certificates

IC 12-14-3-1
Designation of information
    Sec. 1. When assistance is granted to a dependent child underIC 12-14-1 through IC 12-14-9.5, the award made must be entered ona written notice prescribed by the division that designates thefollowing:
        (1) The name and residence of the recipient.
        (2) The amount of the award.
        (3) The date when the assistance is to begin.
        (4) Any other fact required by the division.
