Licensed or otherwise qualified providers; participation inMedicaid program
    Sec. 2. If a provider is required to be licensed or otherwisequalified under Indiana law or the law of another state, the assistancemust be obtained from a provider who is licensed or otherwisequalified and is enrolled to participate in the Medicaid program.

    Sec. 3. The office may by rule require the individual to furnishproviders with information regarding Medicaid provided to theindividual.

    Sec. 4. The office shall provide forms for providing informationunder section 3 of this chapter.

    Sec. 5. The office may by rule require a provider to makeavailable to the office information necessary to document Medicaidprovided under this chapter.

Selection of primary dental provider encouraged
    Sec. 6. The office shall encourage the parent of a child who is arecipient of Medicaid to select a primary dental provider for the childbefore the child is eighteen (18) months of age.
