Required information
    Sec. 2. A provider agreement must include information that theoffice finds necessary to facilitate carrying out this article.

Compliance with enrollment requirements
    Sec. 3. A provider who participates in the program, including aprovider who is a member of a managed care organization, mustcomply with the enrollment requirements that are established underIC 12-15.

Direct access of enrollee to provider
    Sec. 4. (a) A provider that participates in the Medicaid programis considered a provider for both the Medicaid program and theprogram under this article.
    (b) If an enrollee in the Medicaid managed care program forchildren has direct access to a provider who has entered into aprovider agreement under IC 12-15-11, an enrollee in the programhas direct access to the same provider.
